1. Who should have a physical examination?

The overseas students needing to get “X visa” and “Residence Permission for Foreigners” for the course over 180 days should have a physical examination.

2. Where do the overseas students go to have the physical examination? What kind of documents do they need?

The overseas students will go to Chengdu International Hygiene Quarantine Center for the physical examination.

The documents needed in physical examination:

(a) Passport (original copy and its duplication)

(b) 2 pieces of recent passport-size photos

3. Are the overseas students allowed to have the physical examination outside China ?

Yes. The health report from outside of China needs to be verified by Chengdu International Hygiene Quarantine Center .

The documents needed in the verification:

(a) Passport (original copy and its duplication)

(b) 2 pieces of recent passport-size photos

•  All documents of the physical examination from outside China (original copies)

4. What is the relation between the length of the course and visa?

The overseas students for the course within 180 days will apply for the “F” visa. The overseas students for the course over 180 days will apply for “X” visa and “Temporary Residence Card for Foreigners”.

5. What documents are needed in processing short-term visa (“F” visa)?

The following documents are needed in processing short-term visa (“F” visa):
Passport and valid visa (original copy)
Visa Application Form for Overseas Students (Form JW202) (original copy)
Admission Notice (original copy)
Registration Form for Temporary Accommodations (original copy)
1 piece of recent passport-size photo
Application Form of Visa and Residence Card (original copy)
Relevant certification made out by the Foreign Affairs Office (original copy)

6. What documents are needed in processing Residence Card?

The following documents are needed in the procedure of Residence Card.

(a) Passport (original copy)

(b) Visa Application Form for Overseas Students (Form JW202) (original copy)

(c) Admission Notice (original copy)

(d) Registration Form for Temporary Accommodations (original copy)

(e) 2 pieces of recent passport-size photos

•  Application Form of Visa and Residence Card (original copy)

•  Health Certificate (original copy)

•  Relevant certification made out by the Foreign Affairs Office (original copy)

7.  How to apply for the residence permission extension and address change?
    The overseas students who continue the study in China after expiration of the visa are requested to process the extension of the residence card. The overseas students who need to change their living address are requested to process the address change on residence card.
* The overseas students who need to apply for residence permission extension should go to the Overseas Students Department to get relevant certification with their passport and residence card 10 days before the end of the valid date.
* The overseas students who need to apply for the address change should go to the Overseas Students Department to get the relevant certification with their Registration Form of Temporary Accommodations, passport and residence card
